RN Care Coordinator: Full-Time

Crescent Cove is looking for a passionate and skilled registered nurse to serve as a care coordinator. The primary role of the care coordinator is to plan and prepare for respite and hospice care of our patients, including communicating with families and community health care providers,  scheduling care assessments, participating in the development of the child’s care plan, reconciling medications, and arranging and leading admissions and discharges of children and families staying at Crescent Cove for respite or end-of-life care.

Registered Nurse Positions: Full-Time Night and Full-Time Day/Night (Both positions include every third weekend)

Crescent Cove is looking for skilled and passionate nurses to care for kids aged 0-21 at the Midwest’s first children’s residential respite and hospice home. Nurses will facilitate and administer care, as needed, to Crescent Cove children and their families on a comprehensive level including medical, psychosocial and spiritual needs. The nurses, along with their team of supervising nurses, will manage all patient care needs alongside the interdisciplinary team. A good candidate for a Crescent Cove nurses embraces the child and their family, celebrates the child’s life, and helps assure the family while their child is in respite and/or receiving end-of-life care at the Home.

Certified Nursing Assistant: Full-Time and Per Diem (Weekdays and weekends for days and nights)

Crescent Cove is looking for passionate certified nursing assistants with clinical experience to care for children, teens and families at the Midwest’s first children’s residential hospice. This nursing assistants will embrace the child and their family, celebrate the child’s life and help assure the family as their child is in respite and/or hospice at our home. The nursing assistants will facilitate and administer care daily care, as needed, to Crescent Cove children and their families on a comprehensive level including medical, psychosocial and spiritual needs. The clinical assistants, along with their team of supervising nurses and a social worker, will manage all patient care needs.
